CPT 99490 reimburses for 20 minutes of non-face-to-face time delivered by clinical staff. The majority of this time is usually composed of care management services. According to the Centers for Medicare and Medicaid Services (CMS), CCM services that are in scope for this include:
- Systematic assessment of the patient’s medical, functional, and psychosocial needs
- System-based approaches to ensure timely receipt of all recommended preventive care services
- Medication reconciliation with review of adherence and potential interactions
- Oversight of patient self-management of medications.
