CPT 99454 is used to bill for the ongoing supply of Remote Patient Monitoring (RPM) devices and the automatic collection and transmission of physiologic data. This code is billed on a monthly basis and supports continued patient participation in an RPM program.
CPT 99454 may be reported once per calendar month and requires a minimum of 16 days of data transmission within a 30-day period, in accordance with CMS guidance.

- Ongoing supply of connected Remote Patient Monitoring devices
- Automatic digital collection and transmission of physiologic data
- Data transmission that occurs on at least 16 days within a 30-day period
CPT 99454 Billing Requirements:
- Billed once per calendar month
- Requires ≥16 days of physiologic data transmission per 30-day period Applies to ongoing RPM services, not initial setup
- Eligibility and participation requirements for RPM services have not changed
