CPT 99454 Remote Patient Monitoring Device Supply and Data Transmission

CPT 99454 is used to bill for the ongoing supply of Remote Patient Monitoring (RPM) devices and the automatic collection and transmission of physiologic data. This code is billed on a monthly basis and supports continued patient participation in an RPM program. 

CPT 99454 may be reported once per calendar month and requires a minimum of 16 days of data transmission within a 30-day period, in accordance with CMS guidance. 

What CPT 99454 Covers 

CPT 99454 includes: 

  • Ongoing supply of connected Remote Patient Monitoring devices 
  • Automatic digital collection and transmission of physiologic data 
  • Data transmission that occurs on at least 16 days within a 30-day period 

CPT 99454 Billing Requirements: 

  • Billed once per calendar month 
  • Requires ≥16 days of physiologic data transmission per 30-day period Applies to ongoing RPM services, not initial setup 
  • Eligibility and participation requirements for RPM services have not changed
