CPT 99458 is an add-on code used to bill for each additional 20 minutes per calendar month of Remote Patient Monitoring (RPM) treatment management services provided in the same month as CPT 99457. This code is reported when RPM treatment management time exceeds the initial 20 minutes covered by CPT 99457.
CPT 99458 supports continued clinical management of patients enrolled in an RPM program when additional time is required.

CPT 99458 includes:
- Each additional 20 minutes per month of RPM treatment management time
- Ongoing review of RPM physiologic data
- Interactive communication with the patient related to RPM care, as applicable
CPT 99458 Billing Requirements:
- Reported as an add-on code to CPT 99457
- Billed only when CPT 99457 requirements are met
- May be reported multiple times per calendar month, as appropriate
- Applies to ongoing RPM treatment management services
- Eligibility and participation requirements for RPM services have not changed
